Job summary

Moms In Motion - A Virginia Service Facilitation Provider is seeking a Service Facilitator to visit families in their homes and help them navigate Virginia's Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). No degree required; must live in VA and be able to travel locally.

You will write plans of care and assessments, troubleshoot authorizations and timesheets, and build trusted relationships with families while working remotely from your laptop.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience valued over formal education.
• Proficient with email, docs, spreadsheets, portal navigation, and video calls.

Responsibilities

• Visit clients in their homes to support them locally.
• Act as the go-to guide for families navigating waiver programs.
