• Travel up to 40% of time to visit members.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs following the MCP.
• Travel to members' locations for face-to-face social assessments and quarterly visits.
• Arrange non-medical supports including social integration, housing, and community resources.
• Maintain accurate documentation including MCP updates and service authorizations.
