Job summary

Humana is seeking a Case Manager to join the Inclusa/Humana team serving Wisconsin Family Care members. You will work to assess needs, develop a comprehensive care plan, and coordinate services with a Field Care Nurse.

This role requires travel to member residences and regular documentation of progress. Ideal candidates hold a Bachelor's degree in human services or related field, with appropriate experience, and can travel up to 40%.

Qualifications

• Bachelor's degree in human services or related field required with 1 year of experience serving frail elders or adults with disabilities; or a Bachelor's in another field with 3 years of such experience.
• Travel up to 40% of time to visit members; valid state driver's license and 100/300/100 insurance limits.
• Preferred: reside within 45 mins of coverage area; CM experience; electronic case note documentation; knowledge of community health resources.

Responsibilities

• Assess members' health and safety to develop a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to address needs in the least restrictive environment.
• Travel to member locations for enrollment assessments and regular visits; maintain monthly contact by phone.
• Arrange support services related to social integration, housing, employment, and other non-medical needs.
• Maintain documentation including MCP updates and service authorizations.
• Evaluate risk factors and provide member education.
