Job summary

Humana Inc. seeks a Case Manager/Community Resource Coordinator (CRC) to join the Inclusa team serving Wisconsin Family Care members. You will partner with colleagues to provide social service care management for frail elders and adults with disabilities, coordinating plans of care and facilitating access to community resources.

This field role requires travel up to 40% in Jefferson County, WI, with typical hours Mon-Fri 8:00 am–4:30 pm CST and mileage reimbursement for travel.

Qualifications

• Bachelor's degree in Human Services or related field with 1 year experience in Family Care populations OR
• Bachelor's degree in any field with 3 years related experience with Family Care populations

Responsibilities

• Learn what is important to the member and support them in achieving it.
• Partner with a Health and Wellness Coordinator (RN) and collaborate on the member's plan.
• Perform face-to-face social-related assessments for enrollment and every 6 months.
• Coordinate quarterly visits and monthly phone contacts.
• Participate in developing and reviewing the member's plan of care.
• Lead in social, community integration, housing, and non-health issues.
• Educate on prevention/wellness and mitigate risk.
• Document activity and complete paperwork as required.
• Traveling will be required with mileage reimbursement.
