Humana, in collaboration with Inclusa, seeks a Case Manager/CRC to deliver comprehensive social service management for Wisconsin Family Care members. The role involves field visits, collaboration with a nurse coordinator, and addressing non-health needs like housing and employment.

The position requires a bachelor’s degree in Human Services (or related field) with FC experience, with travel up to 40% in Jefferson County, WI. This remote role includes mileage reimbursement and a 40-hour workweek.

Qualificações

• Bachelor's degree in Human Services or related field with 1 year FC experience.
• Alternatively, Bachelor's in any field with 3 years FC-related experience.
• Experience with frail elders and adults with intellectual/developmental/physical disabilities.

Responsabilidades

• Learn what is important to the member and support them in achieving it.
• Collaborate with a Health and Wellness Coordinator (RN) on the member's plan.
• Perform face-to-face social-related assessments at enrollment and every 6 months.
• Coordinate quarterly face-to-face visits and monthly phone contacts.
• Develop and review the member's plan of care.
• Lead in social, community integration, housing, and non-health issues.
• Educate on prevention/wellness and mitigate risk.
• Document activity and complete paperwork; mileage reimbursement.

Conhecimentos

Bachelor's degree in Human Services

Experience with Family Care target Pop
