CVS Health is seeking a Case Management Coordinator to support Aetna members, coordinating care and helping with appointments and benefits education. The role requires home-based work with significant travel within East Central Illinois and strong clinical experience.

You will collaborate with members, providers, and community partners to improve health outcomes, while navigating multiple systems and maintaining HIPAA-compliant practices.

Qualifications

• Residence in Illinois is required.
• Willingness to travel up to 50-75% of the time from East Central Illinois.
• 3–5 years clinical practical experience.
• Independent work via home-based setup and virtual collaboration.
• Strong analytical and problem-solving abilities.
• Effective communication, organizational and interpersonal skills.
• Proficiency with standard office software (MS Word, Excel, Outlook, PowerPoint) and some proprietary applications.

Responsibilities

• Coordinate case management activities to support appropriate healthcare outcomes for members.
• Assist members with appointment scheduling and accessing benefits and education using care management tools.
