Job summary

You will coordinate care plans, evaluate needs, and ensure compliance with care management standards to achieve optimal outcomes. Ideal candidates have at least two years in case management, strong Microsoft Office and EHR experience, and reside in Illinois.

Qualifications

• Must reside in Illinois, Champaign.
• Valid Illinois driver’s license and reliable transportation.
• Willing to travel up to 75% of the time (mileage reimbursed).
• Minimum of two years of case management experience.
• Microsoft Office and electronic health record experience.

Responsibilities

• Evaluate referred member’s needs/eligibility and plan care.
• Coordinate care plan activities and monitor progress.
• Consult with supervisors and medical directors to improve care.
• Escalate quality of care issues and advocate for services.
• Coach members to participate in medical decisions and healthy lifestyle choices.
• Document care in compliance with regulatory guidelines.
