CVS Health Corporation is seeking a Field Analyst in Case Management to coordinate care, educate members, and navigate resources to improve health outcomes. You will engage with members by phone, assess needs, and apply care management principles to support informed decisions.

The role requires strong communication, motivational interviewing, documentation, and organizational skills, with travel up to 50% and a flexible schedule. A Bachelor’s degree is required; a Master’s is preferred.

Formación

• Two or more years of case management, behavioral health, social services, healthcare, or related field.
• Experience with adult/adolescent populations.
• Strong customer service orientation and problem-solving skills.
• Excellent verbal, written, and telephonic communication.
• Demonstrated motivational interviewing skills.
• Attention to detail and documentation
• Commitment to compliance and quality standards.
• Ability to work independently with minimal supervision.
• Travel up to 50% within 60 miles one way.
• Must reside in Louisiana and work a variable schedule.

Responsabilidades

• Conduct comprehensive assessments to evaluate member needs, eligibility, barriers to care, and opportunities for intervention.
• Provide routine care coordination, support, and education to members.
• Utilize motivational interviewing to increase engagement and advocate healthy changes.
• Empower members to participate in their care plans and make informed decisions.
• Collaborate with case managers, supervisors, medical directors, and providers.
• Present cases at interdisciplinary case conferences to support outcomes.
• Identify and elevate quality of care concerns through established channels.
• Assist members in accessing services, benefits, and community resources.
• Demonstrate negotiation skills to secure services for member needs.
• Maintain accurate, timely, compliant documentation of case management activities.
• Meet productivity, quality, and performance metrics with a diverse caseload.
• Support implementation of case management programs, policies, and processes.
• Utilize multiple systems effectively for care and documentation.
• Manage workload independently while aligning with priorities and deadlines.
