Qualifications

• Minimum of two years of experience in case management or related field.
• Experience with adult and/or adolescent populations.
• Excellent verbal, written, and telephonic communication skills.
• Demonstrated motivational interviewing skills and ability to establish rapport telephonically.
• Strong attention to detail with exceptional documentation skills.
• Ability to travel up to 50% within 60 miles one way.
• Must reside in Louisiana.
• Ability to work a variable schedule, including evenings.

Responsibilities

• Conduct comprehensive assessments to evaluate member needs, eligibility, and barriers to care.
• Provide routine care coordination, support, and education to improve health outcomes.
• Utilize motivational interviewing to boost member engagement and healthy changes.
• Collaborate with care teams to address member needs and advocate for resources.
• Document all activities accurately and on time to comply with standards.
• Meet productivity and performance metrics while managing a diverse caseload.
