Qualifications

• Minimum 2 years of case management or related healthcare experience.
• Experience with adult and/or adolescent populations.
• Strong customer service orientation and problem-solving skills.
• Excellent verbal, written, and telephonic communication.
• Demonstrated motivational interviewing skills and ability to build rapport telephonically.
• Strong documentation skills and attention to detail.
• Commitment to compliance with regulatory standards.
• Highly organized, self-motivated, and able to work independently.
• Strong time management and multitasking abilities.
• Proficient with computer systems used for case management and documentation.
• Ability to travel up to 50% within 60 miles one way.
• Must reside in Louisiana.
• Ability to work a variable schedule including two evenings per week.

Responsibilities

• Conduct comprehensive assessments to evaluate member needs and barriers to care.
• Provide routine care coordination, support, and education to members.
• Use motivational interviewing to increase engagement and promote healthy lifestyle changes.
• Empower members to participate in their care plans.
• Collaborate with case managers and providers to address needs.
• Present cases at interdisciplinary conferences to support outcomes.
• Identify and escalate quality of care concerns through established channels.
• Assist members in accessing services, benefits, and community resources.
• Demonstrate negotiation skills to secure necessary services.
• Maintain accurate, timely documentation of case management activities.
• Meet productivity, quality, and performance metrics while managing caseloads.
• Support implementation of case management programs and processes.
• Utilize multiple systems to support member care and documentation requirements.
