Responsibilities

• Present cases at interdisciplinary conferences.

• Assist members in accessing services and benefits.
• Maintain compliant documentation of activities.
• Meet productivity and performance metrics.
• Support implementation of case management programs and processes.
• Utilize multiple systems to support care and documentation.
• Manage workload independently while meeting deadlines.
