Job summary

Qualifications

• Bachelor's degree required in Social Work, Nursing or related health care field.
• 2+ years of clinical case management experience.
• Ability to travel up to 75% within an assigned territory to meet with members and providers.
• Reside in or within commutable distance to Sevier County.

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid beneficiaries.
• Incorporate evidence-based practices into care coordination activities.
• Provide self-management support and patient education.
• Coordinate services across acute, home, chronic, and alternative care settings.
