Job summary

Molina Healthcare is seeking a Care Management professional to support coordination across care teams and providers. You will ensure member progress toward outcomes and contribute to strategies for high-quality, cost-effective care delivery.

Responsibilities include assessments, care planning, and ongoing monitoring, with travel up to 40% depending on state contracts and assignments. Strong communication and organization are essential.

Qualifications

• 2+ years in health care, care management or related settings.

Responsibilities

• Complete member assessments within timelines to determine care coordination eligibility.
• Develop and implement care plans with member, caregivers and clinicians.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document progress toward goals.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and motivate members.
• Identify barriers and provide assistance to address concerns.
• Collaborate with licensed care managers as needed.
• Travel of 25-40% may be required based on state contracts.
