Qualifications

• 2+ years experience in health care, preferably in care management or related setting.
• Licensure/certification required only if mandated by state contract.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources.
• Ability to work independently and manage competing priorities.

Responsibilities

• Complete member assessments per timelines and identify care coordination needs.
• Develop and implement care plans with member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across care domains.
• Facilitate ICT meetings and collaborate with care teams.
• Provide education and motivate change during contacts.
• Identify barriers to care and address concerns.
• Travel 25-40% as required by state/contracts.
