Responsibilities include travel up to 40% locally, performing home visits when needed, and maintaining a proactive approach in diverse settings. Requires prior health care experience and communications skills.

Qualifications

• Minimum 2 years experience in health care, care management, or related setting.
• Clinical licensure and/or certification required only if mandated by state contract or regulatory requirements.
• Valid and unrestricted driver’s license and reliable transportation for travel responsibilities.

Responsibilities

• Completes assessments of members per regulated timelines and determines care coordination eligibility.
• Develops and implements care plans with member, caregiver, and health professionals.
• Conducts telephonic, in‑person or home visits as required.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
