Job summary

Molina Healthcare seeks a Care Manager to support care coordination and collaborate with a multidisciplinary team to deliver integrated member care across the continuum. This role aims to advance member outcomes and cost-effective care strategies.

Responsibilities include completing assessments, developing care plans with members and professionals, conducting visits as needed, and monitoring progress. Travel up to 40% may be required to coordinate with teams and providers.

Qualifications

• At least 2 years experience in health care, preferably care management or related setting.
• Clinical licensure and/or certification as required by state contract or regulations.
• Valid and unrestricted driver's license with reliable transportation for travel.
• Knowledge of community resources and ability to navigate system barriers.
• Ability to work independently and manage time effectively.
• Strong communication and interpersonal skills with diverse populations.

Responsibilities

• Completes member assessments per timelines to determine care coordination eligibility.
• Develops and implements personalized care plans with member, caregiver, and health professionals.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plans, documents interventions, and suggests changes.
• Maintains ongoing member caseload with regular outreach and follow-up.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Assesses barriers to care and coordinates assistance to address concerns.
• Collaborates with licensed care managers as needed.
• Travel 25-40% may be required depending on state/contractual requirements.
