Qualifications

• At least 2 years experience in health care, preferably in care management or related setting.
• Clinical licensure/certification required only if state contract/mandate requires it.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of community resources.
• Ability to work independently and manage multiple projects.

Responsibilities

• Completes assessments of members per timelines and identifies care coordination needs.
• Develops and implements care plans with member, caregivers, and health care professionals.
• Conducts telephonic, face-to-face, or home visits as required.
• Monitors care plans and documents interventions and goal achievement.
• Maintains ongoing member caseload and conducts regular outreach.
• Promotes integration of services to enhance continuity of care.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate and motivate members.
• Assesses barriers and assists members to address concerns.
• Collaborates with licensed care managers as needed.
• 25-40% local travel may be required.
