Job summary

Molina Healthcare seeks a Care Coordinator to support care management for LTSS. You will coordinate with a multidisciplinary team to deliver integrated member care, including home visits when required, and develop waiver service plans with members, caregivers, and providers.

Qualifications

• 2+ years health care experience including LTSS or disability/chronic conditions knowledge.
• Licensure/certification as required by state contract or regulation (if applicable).
• Valid driver's license and reliable transportation for local travel.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits when required.
• Develops and implements care plans and waiver service plans with members/caregivers and providers.
• Monitors care plan effectiveness, documents interventions, and suggests changes.
• Promotes integrated services and coordinates with the ICT for approval/denial of services.
• Assesses barriers to care and supports members with psycho/social, financial, and medical needs.
