Job summary

Molina Healthcare is seeking a care management professional to support coordination of member care across the continuum. You will collaborate with a multidisciplinary team to ensure progress toward outcomes and cost-effective care, including travel to various locales as required.

The role requires 2+ years in healthcare, licensure as needed by state contracts, and strong communication skills. Expect 25-40% local travel and a commitment to quality patient care.

Qualifications

• At least 2 years experience in health care, care management, or related field.
• Valid driver’s license and auto insurance for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision.
• Excellent time-management and communication skills.

Responsibilities

• Completes member assessments per regulated timelines and determines eligibility for care coordination.
• Develops and implements care plans with member, caregivers and providers.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress.
• Maintains ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and assists members to address concerns.
• Collaborates with licensed care managers when needed.
• Travel may be 25-40% depending on state/contract requirements.
