Qualifications

• 2+ years healthcare experience, including LTSS/care management or similar settings.
• Clinical licensure/certification required only if state contract mandates.
• In some states, a health-related bachelor's degree may be required.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and ability to work with diverse populations.
• Experience with substances use disorder populations preferred.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians, and care network.
• Monitor care plans and document interventions and goal achievement.
• Promote service integration across behavioral health and LTSS.
• Assess medical necessity and authorize waiver services.
