Job summary

Molina Healthcare is seeking a Care Manager to support care coordination across the care continuum. You will assess member needs, develop care plans with the member and care team, and monitor progress toward outcomes. Travel to member sites may be required.

You will work with multidisciplinary teams to coordinate behavioral health, LTSS, and community resources, ensuring quality and cost-effective care. Strong communication and organizational skills are essential.

Qualifications

• Minimum 2 years healthcare experience in care management or related field.
• Excellent time management and organizational skills.
• Strong verbal and written communication abilities.
• Ability to work independently and in diverse settings.

Responsibilities

• Complete assessments of members within regulated timelines.
• Develop and implement care plans with the care team and member.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans; document interventions and adjust as needed.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate members using motivational interviewing per Molina guidelines.
• Assess barriers to care and provide care coordination.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
