Molina Healthcare is seeking a Care Coordinator to support LTSS care management across multiple settings. You will collaborate with a multidisciplinary team to coordinate integrated member care and ensure adherence to waiver services and timelines.

The role requires travel 25-40% and a valid driver's license; experience with LTSS and care management is preferred. Proficiency in Microsoft Office and strong communication skills are essential.

Qualifications

• At least 2 years of health care experience, including LTSS and care management.
• Driver's license and reliable transportation for travel.
• Knowledge of community resources and ability to work independently.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office and online portals.
• In some states, 1 year with substance use disorders.

Responsibilities

• Completes comprehensive member assessments within regulated timelines and during in-person visits.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members and providers.
• Monitors care plans, documents interventions, and adjusts as needed.
• Promotes integration of behavioral health and LTSS with community resources.
• Assesses medical necessity and authorizes waiver services.
