Job summary

Molina Healthcare is hiring a Care Manager RN in Kentucky to support LTSS/member care across the continuum. You will perform in-home assessments, develop and monitor care plans, and coordinate with physicians and the care team to meet member goals.

Responsibilities include waiver processes, medical necessity reviews, and ICT collaboration. Travel to member homes is required up to 40% depending on state requirements. Strong communication and organizational skills are essential.

Qualifications

• Active RN license required in state of practice.
• Minimum 2 years healthcare experience with 1 year in care management or LTSS.
• Strong communication, organization and problem-solving skills.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Coordinate waiver enrollment/disenrollment and develop care plans with care team.
• Monitor progress of care plans and adjust interventions as needed.
• Promote LTSS integration and collaboration across care teams.
• Identify barriers to care and facilitate referrals to community resources.
• Travel 25–40% locally as required by state/contractual needs.
