Job summary

Molina Healthcare is seeking an RN Care Manager to support care management and coordination across the care continuum, aiming for quality outcomes and cost-effective member care.

You will conduct assessments, develop care plans with the care team, and perform visits as required. Travel up to 40% may be necessary, with an emphasis on coordinating services across behavioral health and LTSS. Strong EMR/HIPAA knowledge and 2+ years of healthcare experience are essential.

Qualifications

• At least 2 years experience in health care, preferably in care management, or experience in a medical and/or behavioral health setting, or equivalent combination of relevant education and experience.
• RN license must be active and unrestricted in state of practice.
• Valid driver's license and reliable transportation for job-related travel requirements.
• Understanding of EMR and HIPAA.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and demonstrate detail-oriented work.
• Ability to work in diverse settings and adapt to different populations and situations.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.
• Microsoft Office proficiency and ability to navigate online portals.

Responsibilities

• Complete comprehensive assessments of members per regulated timelines and determine who may qualify for care management based on clinical judgment, changes in member health or psychosocial wellness.
• Develop and implement care coordination plan with member, caregiver, physician and other health care professionals.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plan effectiveness, document interventions and adjust as needed.
• Maintain ongoing member caseload with regular outreach and management.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Educate, support and motivate change using motivational interviewing.
• Assess barriers to care and provide care coordination assistance.
• May provide consultation and resources to peers.
• Handle complex cases and medication reconciliation as needed.
• Travel 25-40% locally as required by state/contractual requirements.
