Qualifications

• Minimum 2 years in health care, preferably in care management or related settings.
• Active Registered Nurse license in state of practice.
• Valid driver’s license with reliable transportation for travel requirements.
• Understanding of EMR and HIPAA compliance.

Responsibilities

• Performs comprehensive member assessments within regulated timelines to identify care management eligibility.
• Develops and implements care coordination plans with members, families, and care network.
• Conducts telephonic, in-person, or home visits as needed.
• Monitors care plan effectiveness, documents interventions, and suggests updates.
• Maintains an ongoing member caseload with regular outreach and management.
• Promotes integration of behavioral health, LTSS, and home/community resources.
