Job summary

Qualifications

• Registered Nurse with active, unrestricted license in state of practice.
• 2+ years in health care, care management or related setting.
• Understanding of EMR and HIPAA.
• Able to work independently with minimal supervision.

Responsibilities

• Assess members within regulated timelines and determine care-management eligibility.
• Develop and implement care coordination plans with members and care teams.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document progress toward goals.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across care settings and LTSS.
