Responsibilities

• Completes comprehensive member assessments within regulated timelines and identifies care management eligibility.
• Develops and implements care coordination plans with member, care team, and network.
• Conducts telephonic or home visits as required.
• Monitors progress and updates care plan to achieve goals and address barriers.
• Maintains ongoing member caseload and outreach for care management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
