Qualifications

• Active RN license in state of practice
• Minimum 2 years in health care, preferably care management
• Knowledge of EMR systems and HIPAA requirements
• Ability to operate proactively and maintain detail-oriented work
• Ability to work independently with minimal supervision
• Excellent verbal and written communication skills
• Strong time-management and multitasking abilities
• Proficiency in navigating online portals and databases

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plans with member, caregiver, physician and other professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and updates goals.
• Maintains ongoing member caseload and conducts regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and informal collaboration.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination to address concerns.
• May provide consultation and resources to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel 25-40% may be required based on state/contractual requirements.
