Job summary

Molina Healthcare is seeking a Care Manager RN to support care coordination across the continuum. You will assess member needs, build and monitor care plans, and collaborate with physicians, caregivers, and the care team to promote quality, cost-effective outcomes.

Travel 25-40% may be required based on state and contract requirements. The role emphasizes proactive planning, HIPAA compliance, EMR use, and strong communication in diverse settings.

Qualifications

• Active RN license in the state of practice.
• 2+ years in health care, care management or related setting.

Responsibilities

• Complete comprehensive member assessments within timelines and identify care management candidates.
• Develop and implement coordinated care plans with member, caregiver and providers.
• Conduct telephonic, in-person or home visits as required.
• Monitor progress of care plans and document interventions and outcomes.
• Maintain ongoing member caseload and perform outreach.
• Promote integration of services across behavioral health and LTSS.
• Facilitate ICT meetings and collaboration.
• Educate and motivate members using motivational interviewing.
