Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and coordination across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes and help drive the strategy for quality, cost-effective care.

The role requires assessing members, developing care plans, and performing visits as needed. Travel of 25-40% may be required based on state contracts and regulations.

Qualifications

• 2+ years in health care, care management or related setting.
• Licensure/certification required only if state contract mandates it.
• Valid driver’s license with reliable transportation for travel.
• Knowledge of community resources and support services.
• Strong organization and ability to handle multiple projects.

Responsibilities

• Supports care management activities across the continuum.
• Develops and implements care plans with members and care team.
• Conducts telephonic, in-person, or home visits as needed.
• Monitors progress and documents care plan outcomes.
• Maintains ongoing member caseload and outreach.
• Promotes coordination of services including LTSS and behavioral health.
