Job summary

Molina Healthcare is seeking a Care Management professional to support care coordination across the continuum. You will assess members, determine eligibility for care management, and collaborate with physicians, caregivers, and the member's support network to develop tailored care plans.

Responsibilities include telephonic, in-person, or home visits; ongoing plan monitoring; and coordinating with multidisciplinary teams. A valid driver’s license and ability to travel 25–40% are required.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• Valid and unrestricted driver's license with reliable transportation for travel requirements.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and maintain attention to detail.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes assessments of members per regulated timelines and determines who may qualify for care coordination.
• Develops and implements care plans with member and care team to address needs and goals.
• Conducts telephonic, face-to-face or home visits as required.
• Performs ongoing monitoring of care plan to evaluate effectiveness and document progress.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services to enhance continuity of care.
