Job summary

UnitedHealthcare is hiring a Field Care Coordinator (HIDE SNP) in Wayne County, MI and surrounding areas. This field-based role requires travel up to 75% within a 50-mile radius and offers telecommuting flexibility from a home-based office.

Responsibilities include developing person-centered care plans, coordinating services, advocating for patients, and coordinating with care teams to ensure timely, cost-effective care.

Qualifications

• Must possess one of the following licenses: RN in Michigan or LMSW/LCSW/LLMSW in good standing.
• 2+ years of experience in community health care settings.
• 1+ years working with SDoH and community support organizations.
• 1+ years of experience with LTSS or home- and community-based services.
• 1+ years of experience with electronic documentation systems and MS Office.

Responsibilities

• Develop and implement person-centered care plans across the continuum of care.
• Coordinate and communicate health information to stakeholders for timely, quality care.
• Advocate for patients and families with health care teams.
• Identify barriers to care and initiate appropriate care management interventions.
• Conduct home visits and coordinate with care team and facilities.
