Qualifications

• Must hold one of the licenses listed above.
• 2+ years of experience in community health settings.
• 1+ years with behavioral health providers and SDoH supports.
• 1+ years experience with long-term care or home and community-based services.
• Access to a quiet home workspace with PHI security and ability to travel up to 75%.

Responsibilities

• Develop and implement care plans as a single point of contact.
• Coordinate and communicate health information for quality, timely, and cost-effective care.
• Advocate for patients and families with the health care team.
• Assess and plan individualized care strategies, least restrictive level of care.
• Identify problems and initiate appropriate care management interventions.
• Identify and initiate referrals for social services (financial, housing, etc.).
• Provide resource support for local services (CSHCS, employment, housing).
• Manage the person-centered service plan across the care continuum.
• Conduct home visits and, when needed, in-person visits (nursing homes, hospital, home).
• Document and maintain member information to meet state/federal guidelines.

Education

Registered Nurse (RN) license in Michigan

Social Worker licensure (LMSW/LCSW/LLMSW)

Bachelor's degree in Social Work or related field
