Job summary

UnitedHealthcare is seeking a Field Care Coordinator to manage LTSS for members in Wayne County, MI and surrounding areas. The role combines field visits with a home-based office, traveling up to 75% within a 50-mile radius to ensure timely, cost-effective, and quality outcomes.

The role emphasizes creating person-centered care plans, coordinating with care teams, and addressing social determinants of health through community resources.

Qualifications

• Must hold one of the listed professional licenses in Michigan.
• 2+ years of community health care experience.
• 1+ year of experience with social determinants of health (SDoH) resources and community services.
• 1+ year of experience with long-term care or home- and community-based services.
• Proficiency in electronic documentation systems and MS Office.

Responsibilities

• Develop and implement person-centered care plans across the continuum of care.
• Communicate health-related information to stakeholders to ensure coordinated care.
• Advocate for patient and family needs with the health care team.
• Assess and plan care strategies for appropriate levels of care.
• Identify barriers to care and initiate appropriate interventions.
• Coordinate referrals to social services and community resources.
