Job summary

UnitedHealth Group is hiring a Field Care Coordinator - HIDE SNP in Detroit, MI to develop person-centered care plans and ensure timely access to LTSS services. The role acts as a single point of contact across the continuum of care, coordinating with providers and families to meet individual health needs.

The position requires active Michigan licensure as an RN or licensed social worker, with multi-year community health experience and ability to travel up to 75% within a 50-mile radius.

Qualifications

• Current, unrestricted licensure as an RN in Michigan, or current social work licensure as LMSW/LCSW/LLMSW or LLBSW/LBSW.

Responsibilities

• Develop and implement care plan interventions throughout the continuum of care as a single point of contact.
• Communicate required health-related information to stakeholders to ensure coordinated care.
• Advocate for persons and families to ensure their needs and choices are represented.
• Assess, plan, and implement individualized care strategies.
• Identify barriers to care and provide management interventions.
• Initiate referrals for social service programs.
• Provide resource support to members for local services.
• Manage the person-centered service plan across the continuum of care.
• Conduct home visits in coordination with the person and care team.
• Gather, document, and maintain member information and care management activities.

Education

RN (Registered Nurse) – Michigan Licensure

LMSW/LCSW/LLMSW – Licensed Social Worker

LLBSW/LBSW – Licensed Bachelor Social Worker
