Job summary

UnitedHealth Group is seeking a Field Care Coordinator- HIDE SNP to manage person-centered care across LTSS, including case management and discharge planning.

The role is field-based with a home office and travel up to 75% within a 50-mile radius, primarily Wayne County, MI. Requires active MI RN or social work licensure and 2+ years in community health. Telecommute options may be available.

Qualifications

• RN license (MI) active.
• MSW with LMSW/LCSW/LLMSW license.
• Bachelor's in social work with LBSW/LLBSW license.

Responsibilities

• Develop and implement care plan interventions throughout the continuum of care as a single point of contact.
• Communicate health-related information to stakeholders to ensure quality coordinated care.
• Advocate for patients and families to ensure needs are represented by the health care team.
• Assess, plan, and implement individualized care strategies for appropriate levels of care.
• Identify problems/barriers to care and provide care management interventions.
• Initiate referrals for social service programs and local resources.
