Qualifications

• Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) is required.
• 1+ years of health care or case management experience; familiarity with Medicare/Medicaid.

Responsibilities

• Visit Medicaid members in homes, assisted living facilities, and long-term care settings with 75%-90% local travel.
• Assess needs and develop personalized care plans to meet member goals.
• Coordinate services and monitor the care management plan for outcomes.
• Assist enrollees with grievances, appeals, and accessing community resources.
• Provide information about MCO requirements, services, and benefits.
• Ensure services are delivered in least restrictive settings where possible.
