Job summary

Humana seeks a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care. You will assess members’ needs, build personalized care plans, and link families to resources to improve health and well-being. The role involves 75–90% travel and remote coordination with local facilities.

You report to Care Management, work with an interdisciplinary team, and help ensure services are delivered in the least restrictive setting while guiding members toward optimal health outcomes.

Qualifications

• Bachelor’s degree or professional licensure in relevant field.
• LPN in Illinois with one year experience performing comprehensive assessments.
• Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) eligibility.
• Experience in health care case management or related field is preferred.

Responsibilities

• Visit Medicaid members in homes, assisted living facilities, or care settings; 75–90% travel.
• Assess needs and risks; develop and implement care plans.
• Identify barriers to care and coordinate needed services.
• Monitor progress and adjust plans with interdisciplinary teams.
• Assist enrollees with filing grievances and appeals; provide program information.
• Guide members to community resources and ensure services meet goals.
