Job summary

Humana Inc. is seeking a Care Coach 1 in Northern Pinellas County, FL to visit Medicaid members in homes and facilities, assess needs, and develop member-specific care plans. You will coordinate services, monitor progress, and connect members with community resources to improve health outcomes.

Position requires Florida licensure (RN or LPN) or case management experience, live in the area, and willingness to travel. Valid driver’s license and TB screening are required.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology)
• RN license in Florida with at least 2 years of relevant experience
• LPN license in Florida with at least 4 years of relevant experience
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience
• Prior experience with Medicare & Medicaid recipients
• Must live in northern Pinellas county, Florida
• Intermediate to advanced computer skills including MS Word, Excel and Outlook
• AHCA Fingerprint Required
• TB screening required for Humana TB program
• Valid Florida driver's license and personal vehicle liability insurance
• Ability to work Monday-Friday 8:30am-5:00pm

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities with 75-90% local travel
• Assess and evaluate member's needs to establish a member-specific care plan
• Ensure services are provided in the least restrictive setting to promote well-being
• Plan and implement interventions to meet identified needs
• Coordinate services and monitor the care plan against personal goals
• Guide members and families toward appropriate community resources
• Facilitate interactions with payer sources, providers and care teams as required

Skills

Bachelor's degree + 2 years experience

Medicare & Medicaid experience

Bilingual English/Spanish
