Job summary

Humana is hiring Care Coach 1 in Florida to visit Medicaid members in their homes, assess needs, and build care plans focused on optimal well-being. Expect 75-90% local travel and close collaboration with families and providers.

The role requires degrees or licenses noted above, 2+ years relevant experience, bilingual abilities a plus, and a Monday–Friday schedule with TB and background screenings.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience (Health Services, Social Work, Psychology).
• RN, licensed in the state of Florida with at least 2 years of relevant experience.
• LPN, licensed in the state of Florida, with at least 4 years of relevant experience
• Case Worker with a minimum of 6 years of Health Services/Case Management Experience
• Prior experience with Medicare & Medicaid recipients
• MUST reside in the listed areas within the region
• AHCA Fingerprint Required.
• TB screening is required for this role.
• Driver license and personal vehicle liability insurance required
• Mon-Fri 8:30am-5:00pm expected

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and Long Term Care Facilities – 75-90% local travel
• Assess and evaluate member's needs to establish a member-specific care plan
• Ensure services are received in least restrictive setting to promote well-being
• Plan and implement interventions to meet needs
• Coordinate services and monitor the care plan against personal goals
• Guide members and families toward relevant resources
• Facilitate interactions with payers, providers, and interdisciplinary teams
