Job summary

Humana Care Coach 1 evaluates member needs and guides families toward services to promote wellbeing. The role involves home visits, assessing barriers, and connecting members with quality services to drive health outcomes.

We seek motivated case managers in Northern Pinellas County, FL, with 2+ years healthcare experience or RN/LPN credentials. TB screening and background checks required; 40-hour workweek; travel involved.

Qualifications

• Bachelor's Degree with at least 2 years of relevant health services experience
• RN with Florida license and at least 2 years relevant experience
• LPN with Florida license and at least 4 years relevant experience
• Case Worker with minimum 6 years health services/case management experience
• Prior experience with Medicare & Medicaid recipients
• Must live in northern Pinellas county, Florida
• Intermediate to advanced computer skills (Word, Excel, Outlook)
• AHCA Fingerprint Required
• TB screening and driver's license required
• Work Monday-Friday 8:30am-5:00pm

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, or Long Term Care Facilities with 75-90% local travel
• Assess and evaluate member needs to establish a member-specific care plan
• Ensure services are provided in the least restrictive setting to achieve well-being
• Plan and implement interventions to meet needs
• Coordinate services and monitor the care plan against member goals
• Guide members and families toward resources for care
