Job summary

Humana Inc. is seeking a Care Coach 1 to visit Medicaid members in their homes and care settings across West Palm Beach, Lake Worth, Royal Palm Beach, Riviera Beach and Port St. Lucie areas in Florida.

You will assess needs, develop a member-specific care plan, coordinate services, and guide families toward appropriate resources as part of Humana’s care team. This role requires 75–90% local travel and supports 8:30am–5:00pm, Monday–Friday.

Qualifications

• Bachelor's degree or higher in a related field and relevant health services experience.
• RN/LPN licenses required in Florida with relevant experience.
• Case Worker with 6+ years health services/case management experience preferred.
• Intermediate to advanced computer skills (Word, Excel, Outlook).
• Florida background screening and TB screening may apply.

Responsibilities

• Visit Medicaid members in homes or care facilities; 75–90% local travel.
• Assess and plan member-specific care needs and goals.
• Coordinate services to meet care plans and ensure least restrictive settings.
• Develop and implement interventions to meet needs.
• Monitor progress and adjust plans as needed.
• Guide members and families to appropriate resources.
