Qualifications

Care Coaches must meet one of the following qualifications: Bachelor's degree with 2+ years of relevant experience; RN licensed in Florida with 2+ years of relevant experience; LPN licensed in Florida with 4+ years of relevant experience; Case Worker with 6+ years of Health Services/Case Management experience.

Responsibilities

Visit Medicaid members in their homes, Assisted Living Facilities, or Long Term Care Facilities and other care settings – 75-90% local travel.
Assess and evaluate member needs to establish a member-specific care plan.
Ensure members receive services in the least restrictive setting to maintain well-being.
Plan and implement interventions to meet needs.
Coordinate services and monitor progress against member goals.
Guide members/families toward appropriate care resources.
