Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician to work in the field, visiting client sites or designated locations with occasional office attendance for meetings and training. This role supports MyCare Ohio members with physical health, behavioral health, and long-term services and supports.

The LTSS Clinician coordinates with RN input, identifies high-risk members, and develops care plans, ensuring cost-effective care.

Qualifications

• Requires LPN/LVN, LSW, LCSW or LMSW or license other than RN as per state law.
• Current, unrestricted LPN/LVN, LSW, LCSW or LMSW license required.

Responsibilities

• Identifies members for high-risk complications and obtains clinical data.
• Identifies members who may benefit from alternative levels of care or waiver programs.
• Provides information to RN for verification, assessment and care plan development.
• Coordinates care for members with chronic illnesses in collaboration with the health care team.
• Uses clinical guidelines to guide decisions; nursing judgement is not required.
