Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician in Columbus, OH. This field-based role travels to client sites, with periodic meetings in-office, delivering trauma-informed and person-centered LTSS coordination aligned with state requirements.

Responsibilities include care coordination, assessments, and collaboration with RN and health team to manage physical and behavioral health, social services, and long-term supports for members.

Qualifications

• Requires LPN/LVN, LSW, LCSW or LMSW or license other than RN as per state law.
• Nursing diploma or AS in Nursing or related field with minimum 2 years of experience.

Responsibilities

• Assist in identifying high-risk members and obtain clinical data.
• Coordinate care with RN and health team to ensure cost-effective utilization of benefits.
• Support care plans, assessments, and revisions to meet member needs.
• Collaborate with members and caregivers to coordinate chronic care services.
