Job summary

Activate Care is seeking a field-based Registered Nurse (RN) to travel across Massachusetts for in-person health assessments of dually eligible members. The role focuses on timely documentation and proper handoff to care management, not ongoing case management.

The ideal candidate holds an active MA RN license with an Associate Degree in Nursing (BSN preferred) and a valid driver’s license. Field work requires 70%+ travel and independent home visits with flexible daytime hours.

Qualifications

• Active Massachusetts RN license in good standing.
• Associate Degree in Nursing required; BSN preferred.
• Valid driver’s license and reliable vehicle required.

Responsibilities

• Conduct in-person assessments at members’ homes across MA.
• Document assessment findings in health plan platform and send SOAP notes.
• Submit documentation within 48 hours of each assessment.
• Follow safety guidelines and state mandated reporting; ensure HIPAA compliance.
• Maintain regulatory alignment and complete mandatory trainings.
• Perform other duties as assigned.
