Job summary

Activate Care is hiring a Field-Based Registered Nurse to perform in-person health assessments across Massachusetts for dually eligible members. The role focuses on timely, accurate assessments and documentation, handing off to the care management team.

You’ll travel to members’ homes, work independently, and ensure HIPAA compliance while maintaining high-quality submissions. A MA RN license and at least 1 year of home health experience are required.

Qualifications

• Active MA RN license in good standing.
• Associate degree in nursing; BSN preferred.
• 1+ year home health experience required.
• 1+ year assessment experience preferred; RAC-CT a plus.
• Experience with dual-eligible populations preferred.
• Strong clinical judgment and organization.

Responsibilities

• Conduct in-person home health assessments across MA.
• Document findings in GuidingCare and submit SOAP notes within 48 hours.
• Maintain high-quality, timely submissions and address audit findings.
• Follow safety guidelines and HIPAA/NCQA compliance.
• Manage field scheduling and communicate with care management.
