Job summary

Activate Care is seeking a field-based Registered Nurse (RN) to travel across Massachusetts, performing in-person health assessments for dually eligible members and documenting findings for the care management team. You will use GuidingCare for documentation, submit SOAP notes within 48 hours, and maintain HIPAA and NCQA standards while safely conducting visits in diverse home environments.

Required MA RN license, Associate Degree in Nursing, and 1+ year of home health experience; willing to

Qualifications

• Active Massachusetts RN license in good standing required.
• Associate Degree in Nursing required; BSN preferred.
• 1+ years of home health clinical experience required.
• 1+ years of assessment experience preferred (RAC-CT or similar certification a plus).
• Experience with dual-eligible, Medicaid, or Medicare Managed Care populations preferred.

Responsibilities

• Conduct In-Person Assessments in the Field: Travel to members' homes and engage directly with members to perform required assessments.
• Comprehensive Care Management Documentation: Document assessment findings accurately in GuidingCare and send SOAP notes to Care Manager.
• Maintain High Quality & Timely Submissions: Submit complete documentation within 48 hours of each assessment and resolve audit findings within two business days.
• Uphold Safety & Mandated Reporting Standards: Follow field-safety guidelines and adhere to mandated reporter laws.
• Ensure Regulatory & Compliance Alignment: Adhere to HIPAA, NCQA standards, and mandatory training to maintain access.
