Job summary

Activate Care is seeking a field-based Registered Nurse to travel across Massachusetts, performing in-person health assessments for dually eligible members. The role focuses on timely documentation and accurate data entry into the care management system, with emphasis on safety and compliance.

Applicants must hold an active MA RN license and an ADN; BSN is preferred. Travel heavy fieldwork requires reliable transportation and flexibility for day shifts, with optional evening/weekend slots.

Qualifications

• Active Massachusetts RN license in good standing.
• An accredited ADN; BSN preferred.
• Valid driver’s license and own vehicle with auto insurance.

Responsibilities

• Conduct in-person assessments in the field across MA.
• Document assessment findings in GuidingCare and send SOAP notes.
• Submit documentation within 48 hours and resolve audit findings within two days.
• Maintain safety and comply with reporting laws including HIPAA.
• Travel 70%+ to members’ homes and community settings.
