Job summary

Activate Care in Massachusetts is seeking a field-based Registered Nurse to travel to members’ homes and perform in-person health assessments for a managed care plan’s dually eligible members. The role emphasizes timely documentation and handoff to the care management team, not ongoing case management.

You will work statewide in MA, with 70%+ travel, requiring an active MA RN license, Associated Degree in Nursing (BSN preferred), and a valid driver’s license.

Qualifications

• 1+ years of home health clinical experience.
• 1+ years of assessment experience preferred (RAC-CT or similar).
• Experience with dual-eligible, Medicaid, or Medicare populations preferred.

Responsibilities

• Conduct in-person assessments in members' homes across MA.
• Document assessment findings in care management platform and send SOAP notes.
• Submit documentation within 48 hours and address audit findings promptly.
• Adhere to safety, HIPAA, NCQA, and reporting requirements.
• Perform other duties as assigned.
